HERE is the doc. Called a V317 for transfer or retain of number plate.

(Also shows the bit for the serial number of tax disc )

You don't need the tax discs as they send you new ones if the new paperwork then you send your old one in

DVLA office in Dundee do this there and then... used them many a time until the last time we left it to John Clark and i ended up with my parents plate on my new car

Hopefully they'll give it to you there and then. I didn't and been waiting 8 weeks for the retention certificate to put my plate on my new motor! Postal strike happened at just the wrong time...
