I have TLC until 2009 or 50000 miles

Probably won't get the use of most of it with the little miles i do..but what does it cover?

Do you need to pay for anything over and above when the car goes in for service?

It covers all the standard servicing stuff. The only things you may need to pay for are the likes of brake pads etc. Oh, it only gives you one brake fluid change after two years. Whether you actually need a brake fluid change as often as every two years, I doubt it.

I "think" if you are doing very small miles you will get serviced every two years. What happens to your third service, which would be at 6 years, I've no idea.
